smoothing out moving shot

I have a steadycam-type shot which starts on one side of a room and ends in a close up on a characters face. When I try to smooth it out the beginning looks good but by the time I get to the close up it has that locked on effect on the face where every time he moves his head the whole frame moves. I want to be able to begin the shot with one tracking point and end on another one (not on his face), how do I go about doing this?

There's probably a way to keyframe this, but I haven't done it. I'd do two versions of this with the two tracking points and then try and find a point to cut between the two, possibly cutting to the original shot inbetween and manually adjusting the position and scale of the shot to match the end of the version with the first tracking point and the beginning of version with the second tracking point.
